What Is MOHS Surgery?

Mohs surgery offers the highest cure rate available for skin cancer. By examining 100% of the surgical margins under a microscope during the procedure, our skilled surgeons ensure that all cancerous cells are precisely removed while preserving as much healthy tissue as possible. This meticulous technique not only maximizes the chances of complete cancer removal but also minimizes the size of the surgical site. With a smaller wound, we’re able to achieve a more refined and natural-looking repair, delivering both exceptional medical outcomes and the best possible cosmetic result.

Why Is Mohs Surgery Performed?

This advanced procedure is ideal for treating cancers in sensitive or high-visibility areas such as the face, eyes, ears, where preserving as much tissue as possible is crucial. It is primarily used to treat the most common forms of skin cancer, known as basal cell carcinoma and squamous cell carcinoma. However, Mohs is used to treat other forms of skin cancer, large cancers, and/or recurrent skin cancers.

What Is The Mohs Surgery Process?

The Mohs surgeon then carefully removes the visible portion of the cancer with a small rim of surrounding tissue. Each tissue layer is meticulously mapped to ensure exact orientation on the patient, allowing us to pinpoint any remaining cancer with unmatched accuracy.

The tissue is then processed on-site in our specialized laboratory. This process typically takes up to one hour (larger cancers or more aggressive cancers often take longer). This comprehensive analysis is far more precise than standard frozen sections prepared in hospital settings, which only sample portions of the tissue. By handling every step in-house, North Idaho Dermatology ensures a seamless, thorough, and highly effective treatment process.

Post Surgery Analysis

Once the tissue is processed, the Mohs surgeon at North Idaho Dermatology examines the tissue under a microscope to determine whether all of the cancer has been successfully removed. If there are no cancer cells visible around the entire outer edge and deep margins, then the tumor has been fully excised. The surgeon will then repair (often with sutures) or discuss other options to provide optimal healing and cosmetic results.

However, if any cancer cells remain, the surgeon precisely identifies their location and removes more tissue. This tissue is then processed and examined in the same detailed manner as the initial layer. This process is repeated “layer by layer” until no cancer cells remain, ensuring complete tumor removal with maximum tissue preservation.
